Patient Resources

We know healthcare comes with a lot of logistics. This page brings together the practical information you need, including appointments, your patient portal, billing, and forms, so you can find what you're looking for quickly and get back to what matters.

Preparing for Your Appointment

To help us make the most of your visit, please bring:

  • A photo ID and current insurance card

  • A list of your medications, vitamins and supplements, including doses

  • Relevant medical records, test results or imaging not already sent to our office

  • The date of your last menstrual period, when relevant

  • Questions, symptoms and concerns you would like to discuss

  • Any referral or authorization required by your insurance plan

Please arrive 20 minutes early for your first appointment and notify us as soon as possible if you need to cancel or reschedule.

Your first prenatal visit

Prenatal care supports your health and your baby’s health throughout pregnancy. Your first visit may include a review of your health and pregnancy history, medications, estimated due date, routine testing, nutrition and lifestyle questions, and a plan for follow-up care. Bring a list of questions, there is no concern too small to discuss.
